At Stack Overflow, we had a rule that if one person was remote, then everyone else would be replicate and take team calls at their own desks instead of onsite staff congregating in a room. We felt this was the more respectful approach so that no one feels left out of the subtle queues of onsite folks in a conference room. Not sure that was always the best approach based on context, it is just what we felt helped best integrate remote staff into the culture.
